1 About You

Welcome — this form takes about 5–7 minutes to complete. Your answers help us prepare your CDM documents. Once you have finished, click Generate on the final page — the form will prepare a summary to send us by email. No account or special software is needed.

The information you provide will be used to set up and manage your CDM appointment, and may be shared with relevant project duty holders as necessary. It will be retained as part of your project record in accordance with our privacy policy.

Client Classification

This affects your legal duties under CDM 2015 (Construction (Design and Management) Regulations). If you’re unsure, choose ‘Not sure’ and we’ll confirm this with you.

2 The Property

Correspondence Address

We need a correspondence address for the appointment letter if it differs from the site.

Building Details

Used for initial risk screening — technical verification will follow from the project team.
Count all floors including ground floor. A basement counts as an additional storey. Example: ground floor + first floor = 2 storeys; ground floor + first floor + basement = 3 storeys.

Designations

Within roughly 100 metres — used for initial flood and ecology risk screening.
Party walls are shared walls with neighbouring buildings. This is used for initial screening — your designer will confirm the position.
